These terms apply only to text messages from Resilience Code about completing patient intake. They do not replace our Notice of Privacy Practices.

 

Program. After you give verbal consent by phone to our patient intake team, Resilience Code may send you a text with a link to complete intake (the link is provided by PAI3). This is a one-time intake message, not a recurring marketing program. We may send a small number of follow-ups only to finish that intake or to answer you.

 

Message frequency: typically 1 message; up to a few messages only to finish intake or respond to you.

 

Message and data rates may apply. Wireless carriers are not liable for delayed or undelivered messages.

 

Consent. Agreeing to this text is optional and is only for this intake program.
